We may photocopy your insurance card to ensure we have the most up-to-date information.<\/li><li>Photo identification<\/li><li>Payment for services. Copayments for these visits are for the specialty practice rate.<\/li><li>Current address, phone numbers and other contact information<\/li><li>List of current medications<\/li><li>Your pharmacy\u2019s phone number and address<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-lab-hours\">Lab Hours<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>Tuesday, Wednesday and Thursday: 9 a.m. to 4:30 p.m.<\/li><li>Friday: 9 a.m. to 3:30 p.m.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-cancellation-policy\">Cancellation Policy<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>If you must cancel a procedure, we need 72-hours\u2019 notice or you may be charged a $100 fee.<\/li><li>If you must cancel an office visit, we need 48-hours\u2019 notice or you may be charged a $25 fee.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-office-consultations-and-follow-up-visits\">Office Consultations and Follow-up Visits<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">An office consultation generally is needed before recommending any endoscopic procedures. Office consultation is not needed for a routine screening colonoscopy.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-norton-mychart\"><a href=\"https:\/\/mychart.nortonhealthcare.org\/mychart\/\">Norton MyChart<\/a><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Patients of Norton Gastroenterology Consultants of Louisville can use\u00a0<a href=\"https:\/\/mychart.nortonhealthcare.org\/mychart\/\">Norton MyChart<\/a>\u00a0to view portions of their medical records and communicate with our office.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Review your medications, immunizations, allergies and medical history.<\/li><li>View lab results.<\/li><li>Review health education topics and discharge instructions provided by your physician.<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Stay in touch with your doctor\u2019s office:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Send a request for medical advice.<\/li><li>Send a nonurgent message to your physician.<\/li><li>Do not use\u00a0<a href=\"https:\/\/mychart.nortonhealthcare.org\/mychart\/\">Norton MyChart<\/a> to communicate urgent medical concerns.<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Manage your appointments:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Request or cancel appointments online.<\/li><li>View details of past and upcoming
